Issue
The last buff made it able to disable a melee 100% of the time. Since the punishment is greater than any melee's DPS, this ultimately kills the attacker faster.
Suggestion
Increase recharge or drop duration. It's duration need to be <75% of it's recharge

Issue
If you don't have a skill that removes hexes, you are useless for 16 second.
Suggestion
Reduce cast time to 1 second, make it only apply to next 1...4...5 attacks. reduce damage to 15...39...47

[edit] Not useful in PvE

Issue
10 second recharge and no area of effect make it underpowered in PvE
Suggestion
Split into PvE skill and EITHER give it 5 second recharge OR adjacent area of effect.
