The Bloody Heart is a quest item used in the quest Blood And Smoke. It is dropped by Drogo Greatmane, a centaur boss in the Reed Bog explorable area. After looting the item, the quest Blood And Smoke can be completed by returning to The Falls and giving it to Dark Oak.
