  • Disease is a condition which causes 4 pips of health degeneration to a character. It can spread between creatures of the same kind which are nearby each other. The visual representation is a green mist containing black dots surrounding the affected character. The health bar for the affected character also turns pale green.
  • Disease is the only condition that is spontaneously transferable between creatures. This is the most significant difference between disease and poison.
    • The area of effect for contamination is nearby.
    • You can only be diseased once from each source. Thus if an enemy casts Rotting Flesh and you remove it, you will not become diseased again until the enemy casts Rotting Flesh again.
  • Like poison, disease only works on fleshy creatures.
  • Kind refers to creatures of the same type.
    • To not cause balance issues, nonhuman heroes are counted as human in Player vs Player.
  • A Rune of Purity or the inscription "Cast Out the Unclean" can be used to reduce disease duration by 20%.
  • When entering an area in which creatures are known to use disease, party members should bring a party-wide condition removal (e.g. Extinguish) to prevent an epidemic. In addition, it may be wise to use non-human heroes (if available) such as Ogden Stonehealer, Zhed Shadowhoof, Vekk, Jora, Razah, Pyre Fierceshot and M.O.X and the few non-human henchmen.
  • If you drink from the Chalice of Corruption, you will receive a visual effect identical to disease and 2 pips of health degeneration. The visual effect persists even in towns and outposts (although the health degeneration does not). Both the effect and the degen can be removed by completing the followup quest Masters of Corruption or by abandoning the quest.
