Hex Spell. For 6...10 seconds, every time target foe deals damage that foe takes 100...140% of the damage it causes (maximum 80 damage).

- Pain Inverter follows these general rules:
- Damage dealt to the hexed foe is calculated before damage reduction effects such as Protective Spirit.
- Does not apply to passive damage skills such as Empathy, Backfire, or Clumsiness; however, it will affect Visions of Regret.
- Does not apply to life draining skills such as Vampiric Bite.
- Applies to traps that cause direct damage, such as Spike Trap.
- Applies to retaliation-based damage skills, e.g. Whirling Defense, Retribution, or Holy Wrath.
- The base damage is capped at 80, but there is no limit on total damage. Pain Inverter can deal more than 80 damage by using damage modifying skills, including Frenzy and the Lightbringer title.
- Pain Inverter triggers multiple times if the hexed foe hits multiple targets; each time a creature is hit, the skill produces a separate packet of damage, each of which is capped at 80. This makes PI especially effective in these situations:
- Against high-damage area of effect (AoE) spells (base damage is capped at 80 per affected ally) — examples: Searing Flames and Earthquake.
- Against damage over time (DoT) spells (base damage is capped at 80 per pulse) — examples: Searing Heat and Fire Storm.
- Against high-armor foes, especially bosses, since the damage ignores armor.
- Against blocking skills that deal damage to attackers — examples: Whirling Defense, Flashing Blades, and Gladiator's Defense
- Against foes using skills that generate extra packets of damage on attack — example: Ignite Arrows.
- An obvious way to take advantage of PI's mechanics is to provide the target with lots of fodder for AoE and DoT skills, e.g. by bringing minions, spirits, Asura Summons, Summoning Stones and/or animal companions.
