I recently acquired a Break Hammer of Hammer Mastery, which adds 1 to your Hammer Mastery and does 16-29 damage. Is this the sort of Break Hammer you are talking about? If so, I can tell you how I got it.

I just recently got a gold Sundering Break Hammer of Hammer Mastery while i was fighting the skale at Mehtani Keys.
