The Tattered Girl's Cape used to be found in Stingray Strand in Prophecies. This item no longer drops but some people still have it saved in inventory.

After completing the Primary Vanguard Quests in Guild Wars Eye of the North, go back to Gwen in the Hall of Monuments with the Tattered Girl's Cape in your inventory and she will give you the quest Then and Now, Here and There. The Tattered Girl's Cape will disappear if you accept the quest.

This item, along with the rest of Gwen's memorabilia (Preserved Red Iris Flower, Red Iris Flower, Small Girl's Cape, Gwen's Broken Flute, etc.) can be used only once per character, as the quest Then and Now, Here and There itself is non-repeatable.
