“The Vortex leads back to the mortal realm.

The Vortex is where the ending of the game takes place. When you are in the Realm of Torment the world map is no longer available to look at, all it will show are the outposts and missions within the Realm of Torment. The only way to find your way around the Wilderness of the Realm Of torment is by using the mini map.
When you are near the Vortex you suffer the environment effect Approaching The Vortex.
