Use the following form (only change the areas between the <>):

For example, this code:

Creates this output:
NPCs -- Hard Mode NPCs
Issue: When attacked by a lot of enemies, NPCs do not attempt to flee.
How to fix: Make them run away when they are under attack.
Additional info: None.
Note that there is an optional sixth parameter for categorization. A number (2, 3 or 4) will result in that level heading. The default value of the parameter is 3.
