Titles that encourage /afk should probably be reformulated. One such title that comes to mind is the un/lucky. I think if you were to award 1 lucky point per lvl 21+ kill or 1 unlucky point per lvl 21+ that killed the character you would encourage people to get out and play. Perhaps award the difference of levels in un/lucky points; for example, a character kills a lvl 30 then 10 lucky points are awarded whereas if the lvl 30 killed the character then 10 unlucky points would be awarded. Perhaps take it a step further and add to or multiply the award by the boss kill buff; a +1 would add 1 point and so on, or a +2 would add 2 * award. Consider the death penalty for the unlucky. Imagine players trying to create strategies to play at a constant -60 death penalty. This would certainly cause people to get out and play rather than just /afk the character. The only drawback is the gold sink; not as many people spending the gold to buy lock picks or festival tickets.

Of course you could always just provide a hard mode buff to encourage players to play hard mode more in much the same way as all the other priest/shrine buffs. But providing a normal mode method should be encouraged.
