I quite like Impaled's suggestion, though I think the sac could be a bit too high (though mind you the spell bit is potentially quite powerful). Also, it would synergise well with Blood Magic since you could use Blood Drinker or something to get the Bleeding then throw it onto them with Plague Sending et.c.
